Shared Southern Kennebec Community Health Improvement Planning Vision:

A welcoming, vibrant, diverse community that provides stigma-free access so all may achieve their health goals in a clean, safe, nourishing environment.

To support this broad, exciting vision HCCA provides the following mission.

Mission:
To partner with community health stakeholders to create a collaborative community health improvement plan using the Mobilizing for Action through Planning and Partnerships (MAPP) process. (http://www.naccho.org/topics/infrastructure/MAPP/index.cfm)

Key components include:

  1. 1. Creating opportunities for authentic engagement by formal and informal partners
  2. 2. Selecting a finite number of health indicators that meet the needs of partners
  3. 3. Articulating and activating a community health improvement plan that is shared by all

Next Steps:
Two new assessments:

  • The Community Health Status Assessment
         * We still need a few volunteers to establish the health indicators for our partners
  • Themes and Strengths Assessment
         * We would like 15 minutes on agendas of existing meetings

Community Improvement using the MAPP process:

MAPP stands for Mobilizing for Action through Planning and Partnerships.  MAPP is a community-wide planning process designed to improve the public's health over time. Healthy Communities of the Capital Area (HCCA) and other Healthy Maine Partnerships are working across Maine communities to gain community members' input. 

Why should I be interested in MAPP?

Through the MAPP process, HCCA will:

  • • Connect your organization with others
  • • Serve as a voice for your organization locally and at the state level
  • • Assist you in moving your cause forward
  • •  Help build the public health services framework at the local, county, and state level
  • •  Bring your issues to appropriate decision-makers

 

What is my community doing with MAPP?

Through a grant from the state of Maine, HCCA is charged with leading the implementation of MAPP activities in the southern Kennebec County area. 

During the Fall 2008, HCCA representatives worked with partners to recruit community organizations to join together in December to create a vision to guide the public health planning process. On December 8th a diverse and focused group representing local towns came together and accomplished that goal. The above vision is the result.

In the next step, HCCA representatives will again work with partners (you could be one of them!) to identify strengths and challenges in reaching the vision. Would you like to be a part of this step? Please contact us.
